Before you ask, yes, I did try setting ore gen to 64, made it spawn from 0 - 256, replace quarts, lava, netherrack, and I still cannot find it. I'm using no mods other than this.
Can you show the generation page? Use imgur or some other image hosting site.
I've had this same error. Maybe it has to do with pre-1.16 and post-1.16 nether? IDK
