I need help for a simple procedure

I'm making a custom mod for my server. I need a simple code that makes it so when I right click a certain modded item, it deletes itself and gets replaced by another modded item. Can anyone help?

Start editing the Item go to…

Start editing the Item go to Triggers make a new "When right-clicked" procedure and then replace the main hand item.
