Swords with 3D model

Hi!, I dont find the way to make an item for a custom sword model. I mean, the sword model appears in the inventary being sooo ugly and getting out of the box; so I would like to add an image for the inventary but keep the custom model when the player is holding the sword in one of his hands. I suppose it is possible because minecraft do that with its vanilla weapons.
