How to render an item in a NPC's hand?

I'm makin a few mobs, but I don't know how to make them hold swords from vanilla, 

And making a model of the mob holding each sword seems to be not optimal for game performance.

Is there any way to do this?

And also items from other mods?

This weapon is modeled, and as I said, it's not good for game perfomance
