How to make a block with a custom texture?

Hey there I would like some help getting some custom block textures to work, I have done this before but it was 2 years ago and can't remember for the love of me, any help would be appreciated and if you need me to expand on details let me know and I'll respond as soon as I can, thank you in advance.
