Item loot box

Good day to you. I need help because I am making an MRE. But my attempts have failed so far. To make it simple I would like that once the click on the item is done the item in question disappear and give other item. For example once opened it gives carrots. Does anyone have an idea of how to proceed? Thank you in advance for your answer
