You could add some new natural generating structures in the overworld (maybe with hostile mob spawners and loot chests?) You could also add another dimension, but lots of people add that, so I would suggest you could add a biome instead that adds new trees, foliage, and new mobs.
Make a gun... But don't make any bullets so you have a gun that can ownly be used for pistol whipping.
