The when [number] [thrown item] touches [block] block would work like the if do block where it has a input slot beneath that triggers when a selected item touches a selected block.
The Set items in [Provided itemstack] (thrown item) to [item] block would change the given thrown item into a different item
For an example here's a edited together layout that would change one oak log into one crimson stem when the oak log touches lava
