How to Make a mob Change Mobs Once Clicked With Item?

How do I make a mob change into another mob once clicked with a certain item? I've tried and all it does is despawn the mob, but doesn't spawn the new mob. I'm very confused

How do I make a mob change into another mob when right clicked with a certain item? I've tried but all It does is despawn my mob and doesn't spawn the other one. I'm very confused and would love some help. Thanks! (Above is my procedure)
