how do i make the block model change but keep the items it contains

how do i make the block model change but keep the items it contains

im trying to change the blocks model and keep the item in the block the item changes the blocks model from were its placed but then the item disappers

i put the item/items in

and the model changes

but the item/items is gone

can u help?
