Trigger Problem

Hello, I have a problem with my trigger, I have create a trigger When I right-click with a certain item on a certain block, my mob spawn, Except that even though I've set the condition of having the item in my right hand, it still spawns even without anything else in it.

Also how to upload a screenshot ?
