I am making a custom crafting table with the procedure template given. here are pictures of the procedure and the GUI. When I click the button, simply nothing happens.

Did you linked your procedure to the button of your GUI?
yes
The AND after the OR statement at the bottom requires that the special item be in slot 4 before the IF statement can be true.
