Move player to a custom dimension with the ID of this custom dimension.

Hey,
So my problem is quite basic : I want to make the player change its dimension.
"What is the problem with that?" you're going to ask, well the problem is that I want to move the player to a Dimension with an ID.
I'm making an universal portal and I want to be able to move the player to a custom dimension. All the player has to know is the ID of the dimension.
