How can I create a GUI menu to creating a character for a RolePlay server?

I want to program a RolePlay server. In the first step I wanted to create a menu where you can create your character. In the menu you should be able to enter name, birthday, skin and this should be displayed in minecraft as an identity card. How can I do that?
