3D Blender Model Glitched Texture

Hey! I have 2 questions :

1. When i'm setting a 3D Blender model (OBJ and MTL files) to my item in MCreator, in Minecraft, the texture is glitched.. Can someone help me?

In Minecraft (test environment thingy) :

And 2. Is there a way i can change the model in the item bar from this :

 to a custom 512x512 OR 16x16 image?

I don't really know how to fix that, people generally use Techne or Tabula for making models, but i think i have a solution:

 

Go to the files of your mod (open the jar. with rar. or zip.) and search for a texture folder or png, if you find a PNG replace it with your texture, if you find a folder, just put the texture on the textures folder

@#2 I have the same problem. I made the model in 3DS Max. The program(MCreator) ignores my materials.

I found the problem!

Before clicking the export button in Blender, check the "Triangular faces" button, and normally it should be ok!

But i'm still wondering for the second question...
