(Solves) Crafting table template not working?

I am making a custom crafting table with the procedure template given.  here are pictures of the procedure and the GUI.  When I click the button, simply nothing happens.  GUI

The AND after the OR…

The AND after the OR statement at the bottom requires that the special item be in slot 4 before the IF statement can be true.
