This is a pretty annoying issue. I'm able to make 3 biomes in my custom dimension with no problems at all. But when I add a 4th biome, it doesn't generate in the dimension. I've turned "Generate biome in overworld" off, so I'm not sure what's causing this. I've also set the biome weight higher than two other biomes.
Did you make sure to add your fourth biome to the list of available biomes within the dimension element? I forget to do this a lot, so that may be the problem.
