Item destroyed AND give advancement procedure

Hey guys, in quick words, I am creating advancement, which is given, when item is destroyed with procedure (flint and steel). Any help? And also, how to set custom element icon for the advancement? I want to have enchanted flint and steel icon. Thanks for your help! ;)
