How can I make items spawn with custom names and set enchantments in loot tables?

The title is mostly self-explanatory, however, if you need more context. I want an item to spawn with a preset name different from the actual name, so think when you rename items at an anvil. Same goes for enchantments, just enchantments of my choosing for it to naturally spawn with.
