Help with a custom spawn egg

I created a item that spawns a mob when you right click on block wich is good, however I don,t know how to make it spawn in the direction you are seeing, for now I using a code that says to spawn the mob at "Y: Y+1" and they still suffocating on the blocks because they can't do the same thing on the side of the blocks, can somebody help me
