Better Dimensionchanging

This is for everybody who ever wanted logical Nether and Endtravel

with this mod if you are under a specific heigth, you get teleported to the nether (also works the other way). And if you are over y 1000 you get teleported to the end (can be enabled/disabled by a gamerule)
